InsureBlog: Small Employer Health Insurance Tax Credit

The Small Employer Health Insurance Tax Credit, part of the grand scheme of Obamacare to overhaul the current health care system and make health insurance affordable, is another money hole and dismal failure.

Fewer small employers claimed the Small Employer Health Insurance Tax Credit in tax year 2010 than were estimated to be eligible. While 170,300 small employers claimed it, estimates of the eligible pool by government agencies and small business advocacy groups ranged from 1.4 million to 4 million.

That's fewer than 10% of the estimated eligible groups who said thanks, but no thanks, to a tax credit.


If the government can't even GIVE money away, what does that say about the way they are going to manage health insurance going forward?
One factor limiting the credit?s use is that most very small employers, 83 percent by one estimate, do not offer health insurance. According to employer representatives, tax preparers, and insurance brokers that GAO met with, the credit was not large enough to incentivize employers to begin offering insurance

Well yeah, that's a problem all right. You would think the brainiac's in DC would have considered that. But then these are the same folks that pass laws without ever bothering to read them before they vote on them.

The base of the credit is premiums paid or the average premium for an employer?s state if premiums paid were higher. In 2010, for small businesses, the credit was 35 percent of the base unless the business had more than 10 FTE employees or paid average annual wages over $25,000.

Even a 35% direct tax credit failed to provide enough incentive for businesses to provide health insurance if they did not do so already.


This won't improve in 2014 and there is no reason why it should. Of course, small businesses (those with fewer than 50 employees) are not penalized for failure to provide health insurance. In addition, most low wage employees will get their health insurance through the Medicaid expansion as ordered by Obamacrap.

Of course the states can't cover their current Medicaid costs so one wonders where the money will come in 2014.

Somehow this hope and change thing doesn't seem to be working too well. I hope those who voted for change the last time will have become a bit wiser and educated in the interim.

Rumor roundup: The next iPhone

Apple's?annual Worldwide Developers Conference (WWDC) runs?June 11 through?June 15. As in prior years, the?event will kick off with a keynote presentation?and this, naturally, means that?expectations are high and that many are hoping to hear about the next iPhone.

So let's review iPhone rumors, again, just so that we're up to date on things.

When?will the?next iPhone?arrive?
With the exception of the iPhone 4S (released October 2011), every new iPhone model hit shelves in either June or July. So it's no surprise that many?are optimistic regarding the next-generation Apple smartphone being announced at WWDC. But unfortunately,?many recent reports shoot down the idea?that the next iPhone will be shown off during the WWDC keynote.?

According to CNET, analyst Tavis McCourt of Morgan Keegan said the next-generation iPhone should arrive in the fall of this year.?South Korea's Maeil Business Newspaper backs up this release date and explains that the head of human resources at Foxconn's Taiyuan factory said that the device will become available "around October." (Or at least that's?how?Kotaku's Brian Ashcraft translates the publication's claim.)

Of course, there's a flip-side to every piece of gossip. 9to5 Mac believes?a "reliable source at Foxconn in China," who suggested that the device is set for a summer launch.?Digitimes points out?that "the Chinese-language Commercial Times quoted Daiwa Securities as indicating" that the gadget will be unveiled at WWDC. And then there are all the leaked images of purported iPhone components which are already hitting shelves ....

As I've said before,?my best (reasonably educated) guess is that the next-generation iPhone will arrive in late summer or early fall of this year. (And if you h forced me to pick one, I'd go with the fall?? which would give the current, hot-selling 4S a full year to soak up the sales.)

What will the next iPhone look like?
While the iPhone 4S is virtually indistinguishable from its?predecessor, the iPhone 4, most reports agree that the next-generation Apple smartphone will have a significantly different look.?

Topeka Capital Markets analyst Brian White sent a note describing what he learned about the new iPhone while visiting tech suppliers in China and Taiwan,?writes Business Insider's Jay Yarow: "This will be the most significant iPhone upgrade with a four-inch screen and a new, sleek look that we believe will require a unibody case."?Those at 9to5 Mac add?that they've heard???via mysterious, unnamed sources ??that the next iPhone will?"sport a new, larger display that is 3.999 inches diagonally."

At iMore?? which has nailed several Apple predictions in the past?? folks?disagree with White's prediction that the new iPhone will have a larger display. (Though they do hedge their bets and say that the detail isn't set in stone.)?Reuters goes into the entirely opposite direction?with a report that the new iPhone will use a ridiculously large 4.6-inch display.

Some reports have suggested that?the next iPhone may be made out of "Liquidmetal," a strong, durable and versatile material. But Dr. Atakan Peker, the creator of the material says that this rumor likely won't pan out. He explains that the material is most certainly good enough for the iPhone, but that the manufacturing techniques and infrastructure to implement it properly, and on a large scale, do not exist just yet.

What sort of guts will it have?
9to5 Mac's Mark Gurman says?that prototype versions of the next-generation iPhone are rocking 1GB of RAM, and?TechRadar's John McCann offers?that Sony's new back-illuminated stacked CMOS image sensors would be perfect for the next-generation iPhone as they are ideal for super-slim phones (and the iPhone 4S uses some Sony camera technology already. And we certainly agree with?iMore's Rene Ritchie, who suggests?that the device will be 4G LTE compatible???just like the new iPad.

What did we learn?
Many people have said it before and I'll say it again:?"A rumor's not a rumor that doesn't die."

Canadian carriers flocked to the Galaxy S III before we'd barely even left our seats in London to try it, and it looks like they're also more than a little eager to provide launch details ahead of their American counterparts. Telus and Videotron have both confirmed the leaked June 20th release date, and Telus has gone so far as to reinforce suspicions that LTE models will use the Snapdragon S4 instead of the Exynos 4 Quad -- it expects the phone to use a "1.5GHz dual-core processor," which sure sounds like Qualcomm's latest to us. The same carrier has nailed down pricing at $160 on a three-year contract for a 16GB version and $210 for a 32GB model. Other carriers like Bell and Rogers are still a bit commitment-phobic, although that earlier Best Buy tip makes us think they'll fall into line before long.

Update: Samsung's own release has swung our way, and it's providing a slew of details that should closely relate to the US launch. As expected, that's a Snapdragon S4 inside, although we now know that the 2GB of RAM of the NTT DoCoMo version will at least be making its way to Canada as well. The LTE version for Bell, Rogers, Telus and a handful of regional carriers is known as the SGH-i747, while the fabled SGH-T999 stops at 42Mbps HSPA+ and is the edition that Mobilicity, Wind and Videotron will get, since it supports 1,700MHz 3G. There's no mention of a 64GB Canadian Galaxy S III, so you'll still have to import if you frown heavily upon microSD cards.

'Just do it!' not good enough for cancer patients, researchers say

Exercise generally helps the nation's 12 million cancer survivors, but researchers are still working toward being able to prove, with scientific certainty, that prescriptions for daily yoga or 20 minutes of walking will likely extend a patient's survival.

Understanding specifically how exercise benefits subpopulations of cancer patients is among the big topics at the American Society of Clinical Oncology (ASCO) 2012 annual meeting in Chicago, June 1-5, 2012. Several scientists from the James P. Wilmot Cancer Center at the University of Rochester Medical Center are part of the ASCO discussion, including Lisa K. Sprod, Ph.D., a junior faculty member who is being recognized with an ASCO Merit Award.

"In 15 years we've gone from being afraid to recommend exercise to people with cancer, to having enough data that shows, by and large, it is safe and effective, particularly for relief of treatment side effects," said Karen Mustian, Ph.D., M.P.H., assistant professor of Radiation Oncology at URMC and an exercise psychologist specializing in cancer. "But when a patient walks in the door, he or she wants to know how to tailor exercise to their own situation. Should I bike, walk, or lift weights? Is there anything I should avoid? Can certain exercises lower my fatigue? And the scientific community has not settled into a place where we've developed explicit exercise prescriptions, or can effectively narrow the choices for patients."

Several URMC suggest that knowing how to develop and apply specific exercise programs (dose and timing to achieve a certain outcome) is a realistic goal.

Mustian and Sprod investigated physical activity among older people, a group they describe as often overlooked and underestimated in terms of their ability to live with cancer. On June 2 at ASCO they will present two studies in a session titled, Staying Bold While Old:

Mustian conducted a phase II clinical trial to discover whether six weeks of a home-based exercise plan (walking and resistance bands) improved cancer-related fatigue and strength in 58 men with prostate cancer who were treated with radiation and androgen deprivation therapy.

The mean age of the group was 67. The men had a wide range of fitness levels, from frailty to golfers who were fit and active. A control group that did no exercise was also established.

Researchers measured cardiopulmonary function (through a gold-standard test called Vo2-peak) and muscular strength, and found that all exercisers improved while the control group declined in performance. The most significant information, Mustian said, was that everyone who exercised regularly achieved some added benefit ? even the fittest participants.

A larger study is planned to explore whether the physiological effects of exercise are responsible for improvements, or if a psychological component is also present, such as the personal attention one often gains from an exercise program, or the camaraderie of being with friends at a gym.

Sprod investigated the appropriate amount, type, and intensity of exercise in cancer survivors older than 65, who are also experiencing the natural functional declines associated with aging. The double hit, she said, is an understudied issue.

After analyzing a national sample of 14,887 people, Sprod established that older cancer survivors engage in less physical activity ? even routine activity such as stooping, lifting, and walking ? than people without a history of cancer. This may lead to less independence, a higher risk of the cancer coming back, and reduced survival.

The study raises new questions, such as whether the treatment contributes to less activity, or whether patients and physicians are worried about the safety of becoming physically active during and after cancer treatment, or a combination of the two factors, Sprod said.

In another exercise-related study, Luke J. Peppone, Ph.D., a research assistant professor, investigated the effects of a yoga program on women with breast cancer who were taking aromatase inhibitors, medications that deplete estrogen and often cause severe menopause-like symptoms. A frequent complaint is joint pain and muscle aches, sometimes making it difficult for women to get out of bed or grip a fork and knife.

Peppone said a high percentage of breast cancer survivors discontinue aromatase inhibitor therapy because of side effects, putting them at greater risk for cancer recurrence. His study measured the self-reported quality of life and physical discomfort among 95 women taking aromatase inhibitors and 72 women who were not taking the drug.

Each group took part in a four-week gentle yoga program. The women taking aromatase inhibitors reported a significant reduction in pain, muscle aches, and total physical discomfort, Peppone said. The study will serve as a pilot to launch a larger study of exercise to relieve musculoskeletal pain.

His research, as well as Sprod's study, also was selected for presentation at the Multinational Association of Supportive Care in Cancer meeting in June in New York.

Finally, a controlled study of yoga therapy in middle-aged, mostly female cancer survivors showed that a four-week, customized gentle yoga plan significantly reduced perceived difficulty with memory. Michelle C. Janelsins, Ph.D., research assistant professor at Wilmot, randomized patients to one of two groups: standard follow-up care after receiving adjuvant cancer treatment, and standard care plus a program that consisted of breathing exercises, gentle Hatha and Restorative yoga postures, and meditation.

The yoga group had significantly reduced memory difficulty compared to the standard care control group.

Improved memory also partially contributed to improved fatigue and improved quality of life, Janelsins noted.

In some ways, Mustian said, the exercise-and-cancer dilemma harkens to the days when it was not known what types of chemotherapy and how much should be given to cancer patients. Through rigorous scientific inquiry, physicians were able to refine the way they prescribe and administer chemo ? and Mustian believes the same refinement is possible with exercise.

"I think we do an incredible disservice to people who have just been diagnosed with cancer when we say, 'Just do it!' Mustian added. "Sometimes we see 72-year-olds who are more fit than 55-year-olds, and so it is best to look at functional capacity, and then to be as specific as possible when it comes to personalizing exercise for people with cancer. Otherwise we are sending mixed messages."

Sprod, a research assistant professor, will receive the ASCO Pain and Symptom Management Research Merit Award; she is one of 4 young investigators whose ASCO research presentations were selected for a high impact. All exercise studies were funded through a variety of private and government sources, including the National Cancer Institute.
